Does a hacker or cracker enter an IP address and then passwords to gain entry to your system?

Do they need your IP address or is there a way to enter your computer without an IP address? How does a hacker do it? I want to understand exactly what I am trying to defend against when I protect my computer from security threats other than attached viruses?
